Application for approval of elements of the PhD training component (MED)

Reference is made to the PhD regulations § 7 and Guidelines for calculating course credits.

Please complete the form as accurately as possible and attach the necessary documentation on the last page of this form. Note that a maximum of 20 attachments can be uploaded. You can only upload pdf files, and the total sum of the attachments cannot exceed 128 MB. Contact phd@med.uib.no if you need to upload more than 20 files or if your attachements exceed the limit.

For approval of FORMIDL901 (mandatory dissemination) you must submit documentation for 6 credits at the same time. Other activities in the training component can be approved continuously.
